  On this 8th day of July 1864, personally appeared before me, a Clerk of Count of Record, in the County and State aforesaid, Squire COMBS, aged 26 years, who, being duly sworn, declares, that he is the identical Squire COMBS who enlisted in the Service of the United States at Boonville, in the State of Kentucky, on the twenty fourth day of September 1861, was a Private in Company D commanded by Captain JAMESON in the 8th Regiment of Col ?BAMES?, 8th Reg. Inf.. Ky Volunteers, in the War of 1861, and was honorably discharged on the 3th day of May 1864.  That while in the service aforesaid , and in the line of his duty, he received a Gun Shot wound at the battle of Chicamanga, which entered the right temple, and came out at the left, and in consequense of which wound he is totally blind.  This occurred on the 19th day of September 1863, in the State of Georgia.
He makes his application for a pension, provided by the Act of Congress approved July 14, 1862;  and hereby appoints Fitch, Hine & Fox of Washington D.C., as his lawful attorneys, and authorizes them to present and prosecute this claim, and to receive the certificate which may be issued in satisfaction thereof.  His post-office address is Boonville in the County of Owsley and State of Kentucky and occupation is Farmer.

 Also personally appeared before me William LITTLE, G.W. DANIEL, R.S. FROST, and William NEILE, residents of County of Owsley and State of Kentucky, to me well known as credible persons, who, being duly sworn, declare that they were present and saw said Squire COMBS sign his name to the foregoing declaration, and that they have every reason to believe, from the appearance of the applicant, and their  ___
with him, that he is the identical person he represents himself to be, and that his character and habits are good, and that his occupation is that of a Farmer;  and they further state that they have no interest in the prosecution of this claim.
