September 6th 1864
It is hereby certificed, That Squire COMBS a Private in
the company of Capt. R.B. JAMESON (D) in the Eighth regiment of
the United States Infanty Kentucky Volunteers is rendered incapable of
performing the duty of a soldier, by reason of wounds or other injuries
inflicted while he was actually in the service aforesaid and in the line
of his duty, viz:
Buy satisfactory eveidence and accurate examination, it appears
that on the Nineteenth day of September in the year 1863, the aforesaid
Squire COMBS being engaged Battle at or near a place called Chicamanga
in the State of Georgia he received a Gunshot wound on his head which
entered his right Temple and passed out at the left Temple, said wound
lost __ the sight of both Eyes and rendered said Squire COMBS totally
blind and wholly incapable of obtaining a subsistence and he is hereby
not only incapacitated for military duty but, in the opinion of the undersigned
is: wholly disabled from obtaining his subsistence from manual labor.

Surgeons' Affidavit.

It is hereby certified, That Squire COMBS in the company
of Capt. JAMESON, Company (D) in the Eight 8th regiment of the United
States Volunteers of the State of Kentucky is rendered incapable of performing
the daily duty of a soldier, by reason of wounds or other injuries inflicted
while he was actually in the service aforesaid, and in the line of his
duty, viz: while engaged in Battle at Chicamanga.
By satisfactory evidence and accurate examination, it appears
that on the Twentieth day of September in the year 1863, the aforesaid
Squire COMBS being engaged in Battle at or near a place called Chicamanga
in the state of Georgia he received a Gun shot wound __ right temple passing
out at his left temple in consequence of said Gun shot wound said Squire
COMBS is totally and hopelessly blind and rendered wholly incapable
of maintaining himself by manual Labor, and he is thereby not only incapacitated
for military duty, but, In the opinion of the undersigned is totally disabled
from obtaining his subsistence from amnual labor.

Sworn to and subscribed before me on this 13th day of March A.D. 1865, and I hereby certify that W.H. GLASS M.D., and Samuel JOHNSON M.D., who have made and signed the above certificate are residents of Owsley County, in the State of Kentucky and are personally well known to me. Also, that they are regular practicing physicians and surgeons, and are reputable in their profession, never having heard a question raised, or a doubt expressed to the contrary.
