Claimdant was ____ in Surgeon's Certificate of disability ____ of loss of both eyes from gunshot wound _______ the Battle of Chickamanga.

_________ Chickamanga Sept. 20 1863.  The ______ right temple and came out of the left destroying sight of his eyes.

This civil Surgeons examined him Sept 8 1864 and finds him disabled by gunshot through the head __ ball entering right temple emerging at the left destroying of both eyes.

Biennial examination not re__.

Admitted April 28, 1865, to a Pension of  $8.00 per month, commencing from May 3 1864 to July 4 1864 &  $25.00 per month there after
Disability Total                          Disabled by Loss of sight of both eyes
