Declaration For The Increase Of An Invalid Pension

State of Kentucky, County of Owsley

  On this 1st day of August, A.D. one thousand eight hundred and sevety-four personally appeared before me Clerk of the Owsly County Court the same being a court of record within and for the county and State aforesaid, Squire COMBS aged thirty years, a resident of Booneville, county of Owsley State of Kentucky, who being duly sworn according to law, declares that he is a pensioner of the United States, duly enrolled at the Lexington Pension Agency at the rate of  31.25 dollars per month, by reason of disability incurred in the military service of the United States while a Private Co. D. 8th Regiment Kentucky Vol Infantry that his present physical ___ ___  ___ he believes himself entitled to receive an increased pension;  and that he berewith returns his present pension certificate.

  He further declares that he is disabled in the following manner, to wit:  that he totaly lost both eyes by a gunshot wound whilst in the line of duty and in the service of the United States and is entirely incapacitated for manuel labor and depends on personly attendance of others that he appoints (blank) his true and lawful attorney, to prosecute his claim;  that his residence is No. (blank), in (blank) street, of Booneville county of Owsley, and State of Kentucky;  and his post office address is Booneville County of Owsley State of Kentucky.

Personally appeared John BREEDING, residing at Booneville and Robert EVERSOLE, residing at Booneville, persons whom I certify to be respectable and entitled to credit, and who, being by me duly sworn, say they were present and saw Squire COMBS, the claimant, sign his name (or make his mark) to the foregoing declaration;  that they have every reason to believe, from the appearance of said claimant and their acquaintance with him, that he is the identical person he represents himself to be;  and that they have no interest in the prosecution of this claim.

     Sworn to and subscribed before me this first day of August, A.D. 1876, and I hereby certify that the  contents of the above declaration, &c., were fully made known and explained to the applicant and witnesses before swearing, including the words (blank), erased, and the words (blank), added;  and that I have no interest, direct or indirect, in the prosecution of this claim.

  On this 27th day of August 1874 before me the undersigned Clerk of the Owsley County Court personaly appeared John BREEDING and Robert EVERSOLE when I certify to be respectable and ___ to credit and after being duly swore by me states that they are personly acquainted with Squire COMBS by living a neighbor to him and state from our own knowledge that the said Squire COMBS is Totally Blind in both eyes and that he has to at all times have some person to help him to get about by leading him from place to place and help him about when he nessarily has to go he has to keep a person employed to assist him to get about as above described.  __ each make the foregoing statement from our own personal knowledge and that we have no interest in the prosecuting of this claim.
