CHILI - Constance Y. Urbaniak Day, 59, Chili, died at 5:58 p.m. June 21, 2004, in the emergency room of Saint Joseph's Hospital, Marshfield, the result of a car accident.

Rembs/Kundinger Funeral Home is assisting the family with the arrangements.

Constance Urbaniak Day

Constance Y. Urbaniak Day, 59, Chili, died June 21, 2004, in the emergency room of Saint Joseph's Hospital, the result of a car accident.

A memorial Mass of Christian burial will be held at 11 a.m. Monday at St. Gregory the Great Catholic Church, 3160 S. 63rd St., Milwaukee. The family will receive relatives and friends from 10 a.m. until service time Monday at the church.

Rembs/Kundinger Funeral Home, Marshfield, is assisting the family.

Connie was born Sept. 2, 1944, in Milwaukee, the daughter of Henry and Angeline (Kowalski) Urbaniak. She had been a physical therapist at various locations in Milwaukee, Arizona, the state of Oregon and Thorp.

She is survived by her mother, Angeline Urbaniak of Milwaukee; and her siblings, Ronald Urbaniak, Milwaukee, Elaine (Michael) Simono, Pewaukee, and Virginia (James) Worzalla, Pewaukee.
