Thomas John Polacek, 84, Rib Lake, died Thursday, May 30, 2002, at Rib Lake Health Care Center. Funeral services were held Monday, June 3, at St. John Evangelical Lutheran Church in Rib Lake, with Rev. Roger Sloan officiating.

Burial took place at Lakeview Cemetery, Rib Lake. Pallbearers and honorary pallbearers were his grandchildren and great-grandchildren.
