1.Page number, line number from population schedule
2. Name of insane person
3. City or town of residence, and county
4. Whether an inmate of an institution or day patient
5. Form of disease
6. Duration of current attack
7. Age at last attack
8. Whether person must be kept in a cell
9. Whether person must be restrained
10. Whether person is an inmate in a hospital or asylum
11. Total length of time in institution
12. Date of discharge
13. Whether the person is epileptic
14. Whether the person is suicidal
15. Whether the person is homicidal
For Idiots (mentally retarded) lists the following:
1. Page number, line number from population schedule
2. Name of person
3. City or town of residence, and county
4. Whether person is self-supporting, or partly
5. Age at which idiocy occurred
6. Cause of idiocy
7. Size of person's head (large, small, or normal)
8. Whether ever in a training school, and name of school
9. Time spent in training
10. Year of discharge from training school
11. Whether person is also insane
12. Whether person is also blind
13. Whether the person is epileptic
14. Whether the person is paralyzed. Left or right side?
For Blind inhabitants, lists the following:
1. Page number, line number from population schedule
2. Name of person
3. City or town of residence, and county
4. Whether person is self-supporting, or partly
5. Form of blindness
6. Age at which blindness occurred
7. Whether person is totally blind
8. Whether person is semi-blind
9. Whether person was ever in an institution for the blind
10. Year of discharge from training school
11. Length of time in an institution
12. Whether person is also insane
13. Whether the person is also idiotic
14. Whether the person is also deaf-mute
For Homeless children, lists the following:
1. Page number, line number from population schedule
2. Name of person
3. City or town of residence, and county
4. Whether child's father is deceased
5. Whether child's mother is deceased
6. Whether child was abandoned by parents
7. Whether child was surrendered to an institution by parents
8. Whether the child was born in an institution
9. Year admitted
10. Whether the child is illegitimate
11. Whether the child is separated from his/her mother
12. Whether the child was arrested, and what offense
13. Whether the child was convicted or sentenced
14. Whether the origin of the child was respectable
15. Whether the child was removed from criminal surroundings
16. Whether the child is blind
17. Whether the child is a deaf-mute
18. Whether the child is an idiot
